Ендопротезування кульшового суглоба є поширеним та успішно виконуваним методом хірургічного лікування пацієнтів з кінцевими стадіями коксартрозу або асептичним некрозом головки стегнової кістки. Хірургічна техніка виконання цієї операції розвивається і наразі передбачає як застосування дуже поширеного латерального доступу до кульшового суглоба (Hardinge) так і більш малоінвазивного та новішого – прямого переднього доступу. Передній доступ забезпечує меншу інтраопераційну крововтрату та нижчий рівень післяопераційного болю ніж латеральний доступ. Водночас його використання може підвищувати ризик вивиху ендопротеза та нейропраксії через ушкодження бічного шкірного нерву стегна. Суперечливі результати щодо функціональних наслідків застосування обох доступів обумовлюють необхідність оптимізації реабілітації, яка є важливим аспектом успішного лікування пацієнтів. Прискорена реабілітація є сучасним напрямком, що дозволяє скоротити тривалість госпіталізації, покращити функціональні результати та швидше повернути пацієнта до активного життя. Водночас застосування протоколу прискореної реабілітації, в якому враховано ступінь пошкодження м’язів у разі різних хірургічних доступів не вивчено. Також мало досліджено відновлення функції м’язів стегна у післяопераційний період, зокрема міжкінцівкова асиметрія, що впливає на відновлення нормальної ходьби. Метою дослідження було покращити відновлення стану пацієнтів після ендопротезування кульшового суглоба шляхом вибору хірургічного доступу та оптимізації післяопераційної реабілітації. Наукова новизна отриманих результатів. Уперше обґрунтовано диференційований підхід до реабілітації пацієнтів після тотального ендопротезування кульшового суглоба із використанням протоколу прискореної реабілітації (мультимодальна аналгезія, рання вертикалізації та комплекс вправ фізичної терапії) з урахуванням особливостей відновлення м’язів залучених у процес ходьби, отриманих на основі біомеханічного моделювання ходьби у середовищі OpenSim із використанням м’язовосухожильного елемента Хілла. Уперше комплексно охарактеризовано динаміку відновлення сили та асиметрії окремих груп м’язів стегна після тотального ендопротезування кульшового суглоба залежно від типу хірургічного доступу, що проявлялося у вираженішому зростанні сили відвідних м’язів у разі переднього доступу, а у групі з модифікованим латеральним доступом – згиначів; та у наявності міжкінцівкової асиметрії для всіх досліджуваних м’язових груп, крім розгиначів у разі латерального доступу, протягом 3 місяців спостереження. Уперше проведено переклад та лінгвістичну валідацію україномовної версії шкали Forgotten Joint Score-12 (FJS-12) для оцінювання функціонального стану пацієнтів після тотального ендопротезування кульшового суглоба. Уперше встановлено вищу чутливість валідованої україномовної версії шкали FJS-12 порівняно зі шкалою Harris Hip Score при оцінюванні функціонального стану пацієнтів після тотального ендопротезування кульшового суглоба з використанням прямого переднього або модифікованого латерального доступу, що проявлялося у відсутності міжгрупових відмінностей за шкалою Harris Hip Score через 3 місяці при збереження відмінностей за шкалою FJS-12. Практична значимість отриманих результатів. Результатом дослідження є перекладена та лінгвістично валідована україномовна версія шкали Forgotten Joint Score-12 (FJS-12), яка може бути впроваджена у клінічну практику українських лікарів для оцінювання функціонального стану пацієнтів після тотального ендопротезування кульшового суглоба. Встановлено доцільність використання шкали FJS-12 як чутливішого інструменту порівняно зі шкалою Harris Hip Score у віддалені терміни спостереження. Отримані результати щодо особливостей відновлення сили окремих груп м’язів стегна залежно від типу хірургічного доступу (для переднього доступу – вираженіше зростання сили відвідних м’язів, для латерального доступу – згиначів) можуть бути використані для індивідуалізації програм фізичної терапії після тотального ендопротезування кульшового суглоба. Розроблена програма фізичної терапії із використанням прогресивного силового навантаження та принципів функціонального тренування може бути застосована для прискорення відновлення рухових стереотипів та підвищення якості життя пацієнтів після тотального ендопротезування кульшового суглоба.
Total hip arthroplasty is a common and successfully performed surgical treatment method for patients with end-stage hip osteoarthritis or aseptic necrosis of the femoral head. The surgical technique for performing this operation is evolving and currently involves both the very common lateral approach to the hip joint (Hardinge) and the more minimally invasive and newer direct anterior approach. The anterior approach provides less intraoperative blood loss and a lower level of postoperative pain than the lateral approach. At the same time, its use may increase the risk of dislocation of the prosthesis and neuropraxia due to damage to the lateral cutaneous nerve of the thigh. Conflicting results regarding the functional consequences of using both approaches necessitate the optimization of rehabilitation, which is an important aspect of successful patient treatment. Enhanced rehabilitation is a modern trend that allows you to reduce the duration of hospitalization, improve functional outcomes, and return the patient to active life faster. At the same time, the use of an enhanced rehabilitation protocol that takes into account the degree of muscle damage in the case of different surgical approaches has not been studied. There has also been little research on the restoration of hip muscle function in the postoperative period, in particular interlimb asymmetry, which affects the restoration of normal walking. The aim of the study was to improve the recovery of patients after hip arthroplasty by choosing a surgical approach and optimizing postoperative rehabilitation. Scientific novelty of the results obtained. For the first time, a differentiated approach to the rehabilitation of patients after total hip arthroplasty has been substantiated using an enhanced rehabilitation protocol (multimodal analgesia, early mobilization, and a set of physical therapy exercises) taking into account the features of muscle recovery involved in the walking process, obtained on the basis of biomechanical modeling of walking in the OpenSim environment using the Hill muscle-tendon element. For the first time, the dynamics of strength recovery and asymmetry of individual groups of thigh muscles after total hip arthroplasty have been comprehensively characterized depending on the type of surgical approach, which was manifested in a more pronounced increase in the strength of the abductor muscles in the case of anterior access, and in the group with a modified lateral access – flexors; and in the presence of interlimb asymmetry for all studied muscle groups, except for the extensors in the case of lateral access, during 3 months of observation. For the first time, a translation and linguistic validation of the Ukrainian version of the Forgotten Joint Score-12 (FJS12) scale was carried out to assess the functional status of patients after total hip arthroplasty. For the first time, a higher sensitivity of the validated Ukrainian version of the FJS-12 scale compared to the Harris Hip Score scale was established in assessing the functional status of patients after total hip arthroplasty using a direct anterior or modified lateral approach, which was manifested in the absence of intergroup differences in the Harris Hip Score scale after 3 months while maintaining differences in the FJS-12 scale. Practical significance of the results. The result of the study is a translated and linguistically validated Ukrainian version of the Forgotten Joint Score-12 (FJS-12) scale, which can be implemented in the clinical practice of Ukrainian doctors to assess the functional state of patients after total hip arthroplasty. The feasibility of using the FJS-12 scale as a more sensitive tool compared to the Harris Hip Score scale in long-term follow-up was established. The results obtained regarding the features of restoring the strength of individual groups of thigh muscles depending on the type of surgical access (for anterior access – a more pronounced increase in the strength of the abductor muscles, for lateral access – flexors) can be used to individualize physical therapy programs after total hip arthroplasty. The developed physical therapy program using progressive strength training and functional training principles can be used to accelerate the recovery of motor stereotypes and improve the quality of life of patients after total hip arthroplasty.
